The inner ear consists primarily of the cochlea—Latin: Labyrinthus cochlearis (short form: cochlea)—and the vestibular system. Both are located in a cavity within the temporal bone, are filled with fluid, and each contains sensitive hair cells. The hair cells of the cochlea—more specifically, those of the organ of Corti, which is located within the cochlea—convert incoming acoustic-mechanical sound into nerve impulses. These electrical signals are transmitted to the brain via the auditory nerve.
Station 4: The Brain
The brain decodes and interprets incoming nerve impulses, making it the most important organ of hearing. If the brain is not regularly exposed to auditory stimuli, it may lose its ability to select and categorize sounds, which can lead to a decline in cognitive performance—a serious consequence of untreated hearing loss.

With the cochlea and the vestibular system, the inner ear is a key resource for balance, cognitive fitness, and social participation—all of which are central factors in healthy aging. This is because healthy aging is also linked to good hearing: People with good hearing can better orient themselves and move around in their environment, stay in touch with others, and are more socially engaged and mentally active than people with untreated hearing loss.

The hair cells in the inner ear are highly specialized sensory cells. They convert even the slightest sound waves into electrical signals, which are then transmitted to the brain via the auditory nerve. With every sound wave, the hair cells bend and attempt to straighten themselves against the pressure. If they are exposed to high sound intensity—that is, noise—over a long period of time, they no longer have enough energy to straighten themselves. Once the inner hair cells are damaged, the impairment is permanent.

- Everyday noise: Whether it’s loud music through headphones, street traffic, or attending concerts—noise is harmful, especially when you’re regularly exposed to it without sufficient recovery periods. But even one-time loud noises are dangerous.
- Infections and inflammation: Inflammatory reactions, such as middle ear infections, can lead to dysfunction of the hair cells or the auditory nerve and increase the risk of permanent hearing loss.
- Circulatory disorders:The inner ear relies on a steady supply of oxygen and nutrients. Vascular diseases, high blood pressure, or metabolic disorders can impair its microcirculation and make the hair cells more susceptible to damage.
- Stress and constant noise exposure:Chronic stress or constant sensory overload—such as from persistent background noise—increases the effort required to hear and can contribute to long-term strain on the auditory system.
